When Life and Heart Failure Collide: Finding Balance and Quality of Life
This session explores the essential role of quality-of-life-centered care for individuals living with heart failure and the caregivers who support them. We will discuss practical strategies to assess and address physical, emotional, and psychosocial needs throughout the heart failure trajectory. Attendees will learn how to integrate patient-centered communication, early supportive care, and caregiver partnership into everyday practice to enhance well-being, reduce burden, and promote meaningful living for both patients and their loved ones.
Understanding CardioMEMS: Reducing Symptoms and Improving Life
During this session, heart failure patients and their caregivers will learn about the CardioMEMS device and how it can help reduce symptom burden and enhance day-to-day quality of life. Participants will hear from leading experts who will explain how the device works, who may benefit, and what to expect throughout the evaluation and decision-making process. The session will also feature current CardioMEMS patients who will share their personal experiences and successes.
Voices of Strength: Patients and Caregivers Share their Journeys
This inspiring session features heart failure patients and their caregivers as they share personal stories of resilience, hope, and meaningful successes along their journeys. Through honest reflections and practical insights, they will highlight what has helped them navigate challenges, celebrate progress, and maintain quality of life.
