What is the Heart Failure Patient Foundation?
The Heart Failure Patient Foundation was born from a fundamental need identified by the American Association of Heart Failure Nurses (AAHFN). Recognizing the vital necessity for accessible, evidence-based information for patients, caregivers, and the community, the AAHFN embarked on a mission to bridge this informational gap. With nurses as primary educators for patients and families, the focus remained centered on the core belief that patients are at the heart of everything we do—an ethos echoed by the AAHFN.
During the genesis of the AAHFN nearly two decades ago, a pivotal step emerged—a patient education committee was established. This committee became the driving force behind the creation of educational materials that have served patients and caregivers throughout the years. Their enduring commitment resonates in the materials developed annually during Heart Failure (HF) week, coinciding with Valentine's Day in February, catering to both patients and nursing professionals, equipping them with crucial resources on patient education.
In 2017, an important milestone was reached when the AAHFN board sanctioned the establishment of the Heart Failure Patient Foundation as a 501(c)(3) organization. Marilyn Prasun, a former president of AAHFN (2015-2016), fueled by an unwavering dedication to champion the cause of patients grappling with HF and their families, assumed the role of the Foundation's chairperson.
The Foundation's leadership expanded with the formation of a diverse Board of Directors (BOD), deliberately composed of patients, caregivers, business leaders, and healthcare providers. This diverse representation was integral, ensuring that the BOD authentically reflected the multifaceted experiences of patients, families, and caregivers navigating HF. Members of the AAHFN were invited to recommend individuals interested in and committed to serving on the BOD, emphasizing the pivotal role of patient and caregiver perspectives in steering the Foundation's initiatives.

Through collaboration, advocacy, and a shared commitment to our mission, we strive to ensure that no one faces the challenges of heart failure alone.
